Eligibility Criteria

Pakistani citizen holding a CNIC, Smart NIC, NICOP or B-Form on which the father's name is incorrect, misspelled, or does not match official records.
The correct father's name must be backed by valid supporting documents (father's CNIC, FRC, or educational certificates).
The application must be filed by the CNIC holder; for a minor, a parent or legal guardian applies using the B-Form.
