How to Apply — Step by Step
Download NADRA's prescribed name-correction affidavit (undertaking) from the Pak-Identity portal (id.nadra.gov.pk); enter your CNIC number, the incorrect and the correct father's name, and the reason for correction.
Print the affidavit on stamp paper and get it attested by an Oath Commissioner.
Collect supporting documents — father's CNIC (or death certificate), FRC (Form-B), and an educational certificate showing the correct name.
Visit your nearest NADRA Registration Centre (NRC) and collect a token (online name correction is currently limited, so an in-person visit is usually required).
Submit the CNIC modification form with all documents and complete biometric verification (fingerprints, photograph and digital signature).
Pay the applicable fee for your chosen service type (Normal / Urgent / Executive) — confirm the current rate at the counter or on Pak-ID.
NADRA verifies the request against family records; district offices are directed to resolve name-correction cases within about a month.
Track your application using the token / tracking ID by sending your CNIC number via SMS to 8400 or at www.nadra.gov.pk, then collect your corrected CNIC from the centre or by courier.
