B-Form (Child Registration Certificate)
The B-Form, officially the Child Registration Certificate (CRC), is NADRA's identity document for children under 18. It is required for school admission, passports, family registration, and inclusion in the family record (FRC).

Common mistakes to avoid
- Confusing the birth certificate with the B-Form. The Union Council issues the birth certificate; NADRA issues the B-Form. You need the birth certificate first.
- Parent details not matching. If a parent’s name or spelling differs from their CNIC, correct the CNIC record first to avoid rejection.
- Assuming it is fully online. For a new child record, a live photograph and iris scan are captured at the centre for children aged 3 and above.
How to make sure you are on the official channel
Apply only through NADRA — the PakID app, the official id.nadra.gov.pk portal, or a NADRA Registration Centre (NRC). NADRA never phones or messages you asking for a password, OTP or a fee to “unblock” a record, and no agent can create a genuine record faster than the official process.

How to Apply — Step by Step
First obtain the child's computerized birth certificate from the local Union Council.
Visit your nearest NADRA Registration Centre (NRC) with the parents' CNICs and the birth certificate.
Fill in the CRC (B-Form) application and submit the documents.
Complete the parents' biometric verification; under 2026 rules, children aged 3 and above have a live photograph and iris scan captured.
Pay the fee for your chosen service type.
Collect the B-Form; the child is then linked to the parents' family record (FRC).
